Steps to join the Registry
- Click on New User Registration to view our Consent/Authorization Form. If you accept the terms, you will be allowed to create an account with the Registry.*
- Complete the Questionnaire after you have logged in.*
*For those who prefer "paper and pencil" all the forms (Authorization form, Registration Questions, Questionnaire, and Withdrawal form) can be downloaded and printed. Completed paperwork should be sent back to us at:

Questions you may have
What if I am already a member, but forgot my User Name or Password?
If you forget your user name or password, please do not create a new account. Email Jessica Waxler, the Registry Coordinator and we will assist you. You may also use the Forgot User Name and Password link on the Login page.
What if I think my child or legal dependent is already a Registry member?
If you think your child or legal dependent is already in the Registry, please call or email us before you attempt to register them. We can help you determine whether or not they are in the Registry.
